Please review all of the below information regarding insurance, availability, populations and rates prior to requesting an appointment.

  • Once you request an appointment via the client portal:

    • You will then receive an email from "yourprovider@simplepractice.com (check your junk mail if you can't find it).

    • Complete the requested paperwork in the client portal within 48 hours of your request.  If you do not complete this within 48 hours, your requested time will be released. You may request a new time when you are able to complete the initial paperwork.

    • You will then receive an email confirmation for your appointment once all information is confirmed!
