ARE YOU ACCEPTING NEW CLIENTS

Returning clients - You can schedule via the portal or contact me directly. 

Prospective clients - If you have been referred by someone specifically for my services, please complete this form.

DO YOU OFFER EVENING APPOINTMENTS?

The majority of my available appointments are during the day from 8am-4pm.  My evening appointments are currently full.  You are welcome to schedule for daytime appointments and can be placed on a waitlist for an evening opening as it becomes available.

DO YOU SEE CHILDREN OR ADOLESCENTS?

I do not see children or adolescents.  I only see adults ages 18 and up.

DO YOU SEE COUPLES OR FAMILIES?

I provide individual therapy services only.  However, I can offer some great referrals for therapists who specialize in couples or family therapy.

DO YOU TREAT SUBSTANCE USE/ABUSE?

I do not specialize in the treatment of substance abuse but can offer a referral if you wish.

DO YOU ACCEPT WV MEDICAID?

I am not in network with WV Medicaid and Medicaid does not offer out of network benefits. Some other options for services covered by Medicaid would be Family Services of Marion County, United Summit Center or Chestnut Ridge Center.

WHAT INSURANCE PLANS DO YOU ACCEPT?

I am in network with the following insurance plans.  Please contact your plan directly to determine exact coverage related to your specific plan.

  • Blue Cross/Blue Shield

  • Highmark

  • PEIA

  • Aetna (NOT Medicaid plans aka BetterHealth)

  • Medicare

 

I am NOT in network with other plans including these commonly used local insurance plans listed below.  You can contact your insurance company directly to see if you have "out of network" benefits.

  • Medicaid

  • The Health Plan

  • Aetna Better Health

  • UPMC

  • Other plans not listed.

HOW MUCH WILL A SESSION COST ME?

Insurance clients - You could pay anywhere from $0 - $200 out of pocket, depending on your insurance plan. Rates for services vary from  $100-$200, depending on the type of service and length of service.  

  • Insurance - You will be responsible for what your insurance plan requires as part of your contract with them (copay, coinsurance, deductible) and for any services/charges not covered by your insurance.  

  • Private Pay - You will be responsible for the amount of the service you receive. 

  • Payment plans may be available in times of economic hardship.

Standard rates are as follows (effective January 1, 2021) and will vary based on your specific insurance plan. These rates are subject to change each year:

  • Initial assessment (1 hour) 200.00

  • Psychotherapy Session (55min) 175.00

  • Psychotherapy Session (40-45min) 140.00

HOW LONG DOES EACH THERAPY SESSION TAKE?

Initial sessions will be approximately one hour.  Ongoing sessions will be approximately 55 minutes in length unless a shorter time is warranted.

HOW DO I SCHEDULE AN APPOINTMENT? 

NEW CLIENTS 

  1. Please complete this form.

  2. Await an email from april@intuitivewellnessllc.com

  3. If you are scheduled for an appointment, the following will apply.

    • You will then receive an email from "yourprovider@simplepractice.com (check your junk mail if you can't find it).

    • Complete the requested paperwork in the client portal within 48 hours of your request.  If you do not complete this within 48 hours, your requested time will be released. You may request a new time when you are able to complete the initial paperwork.

    • You will then receive an email confirmation for your appointment!

CURRENT CLIENTS

  1. Please visit the client portal at https://iwllc.clientsecure.me/

  2. Choose "I'm an existing client"

  3. Enter the email you use for your client portal.

  4. Check your email for the direct signin link, which is valid for one time use within a 24 hour period.

  5. Select your requested time and await a confirmation from my office.

I am most easily reached via email.  I am unable to respond to text requests from new clients.

CURRENT CLIENTS

Current clients can reach me most easily via the client portal, text or email.

CURRENT CLIENTS IN EMOTIONAL CRISIS

  • Please leave me a detailed voicemail and I will return your call as soon as I am able.

  • Please go to your local emergency room or call 911 if your situation is life threatening and you need immediate help
